1) Have advisor confirm that all National Membership Requirements are met
2) Have every new member complete the electronic Membership Record Form
3) Have every new member print and sign a Candidate Statement Verification Form and submit it to the chapter advisor
4) Have the chapter advisor send a packet that includes the following items to the National Office
a) a typed list of all new members including first, middle, and last names,
b) the signed Candidate Statement Verification Forms for every new member
c) a single check for all new members’ national membership dues
